Importance of Completing the Questionnaire BEFORE your appointment

As menopause care is complicated, providing complete information on the questionnaire helps me:

  • be prepared for the appointment,
  • saves significant time information gathering.
  • Identifies important things in your medical history that may impact the risks and benefits of hormone therapy (for example: hypertension, cardiac, cholesterol, breast / uterine / ovarian cancer, blood clots / PE, osteoporosis (family history of hip fracture), dementia, early menopause)
  • Be aware of med names and dosages and potential drug interactions

Your initial appointment will be scheduled in advance via zoom  ( I prefer zoom to phone due to privacy regulations, I will use text to let you know when to log on)

There is a 15 min time constraint / information “gathering” / get your notes and observations organized so you have a good sense of how you are doing / consider your questions ahead of time and be prepared / if I need to look things up, request medical records this gives me a heads up / including your pharmacy info (FAX number included) allows me to easily forward your prescriptions to your pharmacy of choice; I do not have time to search for information for your pharmacy during our appointments and ask that you help streamline this process by providing me with the correct fax number. This can be found with a google search or by calling your pharmacy. Save it in your phone so you don’t have to search it every time you fill out an update form. Please do not fill this in with N/A or “I don’t know”. I am assuming you would like to receive your prescriptions and this makes the whole process easier. This is especially important for patients outside the Comox Valley.

The work phone number is used for follow up phone calls. It will also be used to text you regarding appointment logistics ONLY. This helps me communicate with you when I am running behind.

FAQ

What is the referral process?

Dr. Wayne is a specialist and therefore appointments can only be made by referral (MSP guidelines). Referrals can come from a family physician or nurse practitioner. MSP does not allow referrals from naturopathic physicians at this time. Referrals can be faxed to 250-871-7375. Your referring physician or NP can also find us on Pathways.

We do 100% of our communication with patients via email. We use an encrypted email service called Hushmail. We value your privacy and understand that your personal medical information requires protection. Hushmail meets all privacy regulations for healthcare and requires you to create a password. Yes. I will follow up with you until your hormone therapy is stable. At that point, if you are staying on HRT, I will refill your prescriptions for a year and send a note to your referring physician noting that you have been discharged from care. After that point, you will need to referred if you wish to see me again. We do our best to offer priority to previous patients.

Follow up appointments are not booked in advance. They are booked on an as needed basis and are triaged in terms of urgency. I will discuss reasons for follow up and a likely time frame during our consultation. If I can answer your questions and suggest adjustments via email, I will do so. When you are wanting follow up, you reach out to Mariah via email and she will send you our update form to be filled out and returned. The information that you provide on this form helps us to determine the method and time frame of follow up so I ask that you give it your time and attention.

No. Absolutely not. I do not have the time or space to book appointments for this reason. I am happy to update your prescriptions between appointments if needed. Before making a request for a prescription: confirm that you actually need refills (I typically write my prescriptions for three months with three repeats). When you email for a prescription, please include the following information: your first and last name, medication and dosage, pharmacy that you would like me to send it to (including fax number). Please do not wait until you are out as these requests can sometimes take up to 2 weeks. Please try to be proactive so that your emergency does not become my emergency.

All requests go through Mariah unless I have emailed you directly. Mariah is cc’d in all email correspondence so that it is file in your medical records for my reference. Mariah triages the emails and brings tasks/questions to my attention. Delays in response are ALWAYS me. Mariah will have an auto responder letting you know she is away if there is to be a delay on her part. We work part time and do not respond to questions / requests immediately. Sending multiple emails about the same thing is not helpful and actually slows us down.
